Too posh to push? More C-sections on demand in UK (AP)

LONDON ? Pregnant women in Britain, where the government provides free health care, may soon be able to get a cesarean section on demand thanks to a rule change that critics describe as the health system caving into the "too posh to push" crowd.

Currently, British women who can't afford to pay private doctors for their baby's delivery have been allowed to have planned C-sections only if there are health concerns for mother or baby. Emergency C-sections are done when the situation demands it.

But new guidelines set to take effect later this month say pregnant women "with no identifiable reason" should be allowed a cesarean if they still want it following a discussion with mental health experts.

"It's about time women who have no desire to view labor as a rite of passage into motherhood be able to choose how they want to have their baby," said Pauline Hull, who has had two children by cesarean because of medical reasons. "The important thing to me was meeting my baby, not the experience of labor."

Hull runs the website, Elective Cesarean, from her home in Surrey, south of London. She said midwives tend to overexaggerate the risks of C-sections and underestimate those of vaginal births.

The new draft guidelines come from the National Institute for Health and Clinical Excellence, or NICE. The agency's guidelines are usually accepted by the government and determine what will be paid for by its health system.

"In general, a C-section is a safe operation, especially when performed as a planned procedure," the new guidance says.

The agency says it routinely updates guidance every few years and denies there was any pressure to change its more restrictive C-section advice. But in recent years, advocates and some doctors have slammed the U.K. health system for not giving women a greater say in childbirth.

The change comes at a price for Britain's cash-strapped health system. NICE estimates C-sections cost about 800 pounds ($1,280) more than a vaginal birth, although that doesn't include the price of treating possible long-term complications like urinary incontinence from vaginal births.

The report notes that for every percentage point the C-section rate falls, the health system could save 5.6 billion pounds ($8.9 billion).

In the U.K., about 25 percent of women have C-sections, versus about 30 percent in the U.S. In both countries, rates have doubled in recent years, though doctors say that's not just due to demand, but because pregnant women increasingly have other problems like obesity and diabetes.

About 10 percent of all U.K. births are planned C-sections while about 15 percent are emergency procedures, according to NHS figures.

The World Health Organization has previously said wealthy countries should aim for a C-section rate of about 15 percent, though it also says there isn't enough evidence to know what the ideal rate is.

The National Health Service estimates that about 15 percent of British births take place in private hospitals, which tend to have higher cesarean rates. At Portland Hospital in London, where many celebrities check in, the C-section rate ranges from 35 to 40 percent.

The issue of women having C-sections on demand has long been a hot button issue in the U.K., with celebrities like Madonna and former Spice Girl Victoria Beckham having scheduled procedures at upscale hospitals. Criticism for the wealthy getting these elective surgeries led to the phrase "too posh to push."

But some experts say the new British guidance won't dramatically change how pregnant women are treated.

"It's only a small percentage of women who ask for a C-section," said Cathy Warwick, chief executive of the Royal College of Midwives.

Warwick said doctors and midwives regularly talk to women who have concerns about childbirth and after addressing their fears, most women agree to skip a planned C-section.

"As long as it's safe for both mother and baby, a vaginal birth is absolutely the best way for anyone to deliver," said Dr. Daghni Rajasingham, an obstetrician and spokeswoman for the Royal College of Obstetricians and Gynaecologists. She said the physical stress put on a baby's lungs during labor helps them adapt to breathing after being born.

Rajasingham also said while C-sections are safe, the operation comes with risks including infections, bleeding, and the potential for problems with future pregnancies.

"As an obstetrician working with limited resources, I want to make sure we have safe and high-quality health care for all women and not be skewed by a few women who want something specifically," Rajasingham said.

For women like Hull, however, the policy shift is long overdue.

"Women shouldn't think a C-section is going to be a walk in the park, but they should have all the information they need to help them make an informed choice," she said.

Cancer Risk Doubles After Organ Transplant, Study Finds (HealthDay)

TUESDAY, Nov. 1 (HealthDay News) -- Organ transplant recipients in the United States double their risk of developing cancer compared to the general population. And that risk is elevated for 32 different types of cancer, according to new research.

In any given year, however, the risk of developing cancer after a transplant is just 0.7 percent. And experts say the benefits of transplantation far outweigh such risk.

"People need to understand that transplantation is one of the great success stories of medicine. It's a very effective treatment for people with severe organ disease," explained the study's lead author, Dr. Eric Engels, a senior investigator in the infections and epidemiology branch of the division of cancer epidemiology and genetics at the U.S. National Cancer Institute in Rockville, Md.

"Our study is saying that this population has a unique pattern of cancer risk. Transplant recipients need to be carefully screened and followed," Engels added.

Results of the study are published in the Nov. 2 issue of the Journal of the American Medical Association.

In 2010, almost 30,000 solid-organ transplants were performed in the United States, according to background information in the article. Kidney transplants accounted for more than half of that total, followed by livers, hearts and lungs.

After transplant, the recipient must take powerful immune system-suppressing medications to prevent the new organ from being rejected.

But those medications put transplant recipients in a catch-22 situation, said Dr. Darla Granger, director of the pancreatic transplantation program at St. John Hospital and Medical Center in Detroit. "Suppressing the immune system does increase the risk of cancer. And, if you have cancer, you want a strong immune system to fight the cancer," she added.

Another issue with the immunosuppressants is cancers that are related to viruses. For example, non-Hodgkin and Hodgkin lymphomas are linked to the Epstein-Barr virus, while cervical and several other types of cancers are caused by the human papillomavirus, and some liver cancers are caused by hepatitis B or C viruses.

"We've always known that certain tumors are increased after transplantation. Certain tumors are known to be related to viruses, so when we give immunosuppressant drugs, we're decreasing the body's ability to fight off viruses," explained Dr. Lewis Teperman, chief of transplant surgery at NYU Langone Medical Center in New York City.

Not all of the cancers can be linked to immunosuppression, however. In some cases, especially with liver and lung cancers, it's possible that a tiny cancer was present in the body before transplant.

"It's hard to sort out the exact cause of cancer, but some are clearly related to being immunosuppressed," Granger said.

Engels' study reviewed data from nearly 176,000 solid organ transplants done between 1987 and 2008.

The investigators found the overall incidence of cancer was 2.1 times higher than would be expected in a non-transplant population.

The risk of non-Hodgkin lymphoma was increased more than sevenfold. The rates of lung and liver cancer were also significantly increased, but Engels said this may be due to previous cancers. For example, a treatment for some liver cancers is a liver transplant, and it's possible that some cancer cells may survive the transplant process. The risk of lung cancer was highest in those who received a lung transplant, and the risk of liver cancer appeared to be elevated only in those who received a liver transplant.

The incidence of kidney cancer was increased in all transplant recipients by almost five times, according to the study. The researchers said this might be due to the underlying disease process in people who need new kidneys, and that the immunosuppressants likely play a role for all transplant patients.

"This study raises some very good points. It suggests that screening for viruses should be done, and that we should always be trying to use less immunosuppressants. It also raises the inclination to screen transplant recipients for tumors," Teperman said.

"This is an important paper, but I think it may overstate the risk. I think the risk of cancer is elevated, but it's probably less than seen here," he added.

For someone who's had a transplant or is waiting for one, Granger noted, "decrease the risk factors you can. Don't smoke. Follow good health practices. Wear sunscreen. And, if you've had a transplant, get the screenings your doctor recommends."

NEW YORK (AP) ? Actress Zooey Deschanel (ZOH'-ee deh-shuh-NEHL') and her husband, musician Ben Gibbard, are breaking up.

Deschanel's publicist Lauren Auslander confirmed Tuesday the couple has separated after two years of marriage. Auslander describes the split as "amicable," with "no third party involved."

Deschanel's career is on a high note with the success of her Fox sitcom "New Girl." Her band She & Him just released a Christmas album, and last month she sang the national anthem at the World Series.

Gibbard is the lead singer of the rock band Death Cab for Cutie.

News of the separation was first reported by Us Weekly.

Winner of Taiwanese "Emmy": "I'd like to thank my iPhone" (Reuters)

LOS ANGELES (TheWrap.com) ? During the recent Golden Bell Television Awards last month in Taipei -- Taiwan's equivalent of the Emmy awards -- Huang Ming-chan, who won one of the top prizes in the director categories, gingerly walked up to the stage and snapped a cellphone photo of himself (over the shoulder) while approaching the podium.

The 40-something director then took another cell phone from his other jacket pocket and started reading congratulatory email messages and smiling to himself, totally ignoring the audience before finally addressing the 5,000 people in the auditorium -- and on national TV -- by reading his acceptance speech from the screen of his iPhone.

Huang's Golden Bell acceptance speech might just have made international televised award-show history, by not only replacing a prepared speech on paper with a written screen text on his phone. For snapping his own self-congratulatory photo as he was handed the award, he deserves some other kind of prize, too. But what should it be called?

Said Sophie Chen, a 25-year-old graduate student in communications at Chung Cheng University in south Taiwan: "I think it was okay for him to take his own photo as he got the award on stage; that's cool, but ignoring the audience for a few seconds while he rather arrogantly checked his own phone messages on stage seems a bit impolite to me. Reading his speech from his phone screen was also cool, I thought. I'm sure this will start happening at the Oscars next year, too. The technology is here already!"

Is this a new trend just getting started, and could it swamp the Oscars telecast in 2012? Stay tuned -- on your iPad!

Rep. Jackson expects to be cleared in ethics probe (AP)

KANKAKEE, Ill. ? U.S. Rep. Jesse Jackson Jr. is predicting he'll be "vindicated" by an ethics investigation into whether he or someone on his behalf offered to raise funds for ex-Illinois Gov. Rod Blagojevich in return for an appointment to President Barack Obama's vacated Senate seat.

"Let me be clear. I believe in the American system of justice," Jackson, a Democrat, said Saturday at a Kankakee County NAACP dinner. "The process is continuing, but in the end I believe I will be vindicated."

The House Ethics Committee announced earlier this month that it would resume a probe that began before Blagojevich's trial. The panel had agreed to abide by a Justice Department request to take no action in the investigation, which is normal practice when the department is concerned a congressional inquiry interferes with its own investigation.

Jackson, who has not been charged, has repeatedly denied wrongdoing.

The congressman has acknowledged he was "Senate Candidate A" in the Blagojevich criminal complaint, one of several candidates whom authorities say the former governor considered for the Senate seat.

In June, jurors at Blagojevich's retrial convicted him on 17 of 20 corruption counts, including trying to sell the Senate seat. He is awaiting sentencing. Related charges against his brother, Robert Blagojevich, were dropped after the first corruption trial last year.

Last week the Chicago Sun-Times reported that Robert Blagojevich, a Tennessee businessman, wants to testify before the House committee. He told the newspaper that he had written to committee members offering testimony and that Jackson needs to answer questions.

Trial witnesses have alleged Jackson supporters offered fundraising for the governor if Jackson became senator. Jackson testified at Blagojevich's retrial that he "never directed anyone to raise money for another politician."

Jackson is seeking re-election in the newly-drawn 2nd Congressional District.
